1)      The producer, importer, seller, repairer or service provider in weights and measures must act follow the regulation and inform to the official staff of central office or local office.

   2)     The officer whom gets the request from no. 1 will examine following the regulation and launch the allowance form to verify business operation within 7 days.

   3)     In case of any mistakes of request, the officer will inform to review within 7 days.

   4)     In case of review within the duration in no. 3, the officer will launch the certificate to the requester within 3 days.

   5)     In case of losing or damaging of business operation document, the substitute document will be issued within 15 days.

To request the certificate in business operation for producer, importer, seller, repairer, and providing services in weights and measures, it need to contact the officer at the Office of Weights and Measures or local office following the form of ชว. 001 and the form of business operation contains of;

   1)     Form CBWM 001 (2 pages) Form of Business Operation in being producer, importer, seller, repairer, or providing service in weights and measures.

          -     Copy of House Registration Form (general person or business)

          -      Copy of House Registration Form indicating the location of company (general person or business)

          -      Copy of Certificate in Official Business (business) not exceed 1 years (case of general person)

          -      Copy of Certificate of Allowance to set a business (official person)

          -      Certificate of Authority (in case of assignment) and copy of identification card of both authorizer and authorizee.

          -       For a business of being producer, importer, seller, and repairer, the identity sign must be requested.

   2)     The sample of 4 identity sign (case of producer, importer, seller, and repairer)

   3)     The document for consideration in location is done for only producer or repairer.

   4)     The document of business operation (2 pages)

   5)     Customer lists

   6)     Tools list

   7)     Allowance of using building

   8)     The request to verify the validity of weights and measures (1 page) and request form to verify validity
